Please read the form carefully. Note that some clubs are capped at a certain number of students and are decided on a first come, first serve basis. This will be decided via timestamps on submission responses. This form will stay open until next Tuesday, September 29th at 5:30pm.

*Club Brochure has been fixed to reflect more accurate information/updates on dates and grade levels.
**Chorus is on the Form but it's the 6th bullet down in the information at the top! It will take you to a new form for chorus.
***This is for Fall and Winter clubs ONLY. Spring clubs will come out in a few months. If you see a club advertised for Spring on the Brochure, don't worry! We will make sure a form gets sent out for Spring clubs later.
